1)  Fill in your full name.
    2)  Fill in your address only if you want your photo is to be mailed to you.
    3)  Give us your contact info, just in case we need to get a hold of you.
    4)  Tell us when the photo was taken.
    5)  What club/event/venue was the photo taken at?
    6)  Describe the photo(s) in as much detail as possible to ensure that the correct photo is printed.
    7)  Fill in the name of the photo. When you click on the photo, please type in the filename of the photo, which begins with the letters DSC_.
    8)  We only accept cash (can be dropped off at the office *) or transfer via Paypal (paypal address: riatnight@aol.com).  Photo must be paid for before we print it out. 
    9) Once payment is received, there are three different ways you can get your photo: 
        (a) I want the photo mailed to me; (b) I'll pick it up at the office; (c) I'll pick it up when I see you guys at the club.
